His bag was earlier recovered from a stream.
After a two day search, the team carried him over rugged terrain for two nights.
One rescuer reached Qalandarchi Fort to get official help. A stretcher team then moved Afzal to Misgar and Sost for treatment.
Afzal was travelling with five companions, who were detained by Chinese authorities for illegal border crossing.
Afzal escaped back to Pakistan but alleged his group stole his phone and cash, abandoning him.
Police are investigating, with one official suggesting the group was lured by job promises across the border.
Afzal is now in police custody, receiving medical care for dehydration and exposure.
Doctors say he is stable but weak. The Misgar community’s quick response earned praise from local officials for saving his life.
